ADMISSION CRITERIA

CRITERIA FOR SERVICES 

Destin Pathways Non-Centered Based Day Support Prosram welcomes evervone who scoks its services. without discrimination based on race, color, creed. gender, or financial status. To be eligible, applicants must be at least 18 years old, with a primary diagnosis of intellectual/developmental disability, and may also have a diagnosis of mental illness. They must not pose a danger to themselves, others, or property. While Destin Pathways can accommodate individuals with physical disabilities such as cerebral palsy and those who use a wheelchair, we are unable to provide extensive licensed medical nursing services for conditions like brittle bone disease or G-tube requirements. 

CONTINUED STAY 
At each Person’s Support Plan meeting, which happens at least once a year, every person will be assessed. The purpose of this evaluation is to determine whether the services provided are suitable and aligned with their perh needs and preferences. The PLAN FOR SUPPORT will also include pre-discharge planning with specific criteria. The progress of each participant will be monitored daily, and this progress will be documented in their individual record. Every four months, goals and objectives will be reviewed to make appropriate adjustments to the PLAN FOR SUPPORT. This quarterly report will be filed in the permanent record and indicate whether continued service is necessary.

EXCLUSION 

Person who fulfill the admission requirements but have not exhibited behavior that poses a threat to the safety and well-being of others, including two or more recorded incidents of physical or aggressive conduct (such as hitting, kicking, spitting, slapping. biting. grabbing, pulling. pushing, or property damage) within a month prior to their enrollment in community-based services, will not be discharged from the program. TERMINATION OF SERVICE AND DISCHARGE OR TRANSITION 

Persons may be terminated from Destin Pathways Non-Centered Based Day Support Program for a varicty of reasons. including problematic bchaviors and personal decisions. If there are any unclear reasons for termination please reach out to the, senior program, Director or program Director for clarification.
